Graceful Hands Community provides Home and Community-Based Services (HCBS) designed to support individuals with developmental and acquired disabilities to live meaningful, self-directed lives in their homes and communities.

Shared Living

A family-style, community-based living arrangement where an individual lives with a trained Shared Living provider who offers daily support, guidance, and companionship in a home environment.

  • 24/7 availability of a Shared Living provider

Residential HCBS

Supportive residential services for individuals who need assistance in a structured setting while maintaining independence.

  • Support with meals, hygiene, and routines
  • Community-based (non-institutional) settings

Community Participation Services
